Dieulafoy病的诊断与治疗(附18例报告)

目的：探讨Dieulafoy病的内镜诊断与治疗。方法：回顾性分析18例引起上消化道出血的Dieulafoy病的临床表现，内镜下特征，治疗方法与效果。结果：18例中有13例（72.2%)为1次检查确诊，5例为2次或2次以上检查确诊；其中15例经内镜止血成功，首次治疗成功12例（66.7%)，3例经2次内镜止血成功（16.6%)，3例需外科手术（16.6%)。结论：在Dieulafoy病的诊断及治疗中，内镜为首选方法。

THE DIAGNOSIS AND TREATMENT OF DIEULAFOY DISEASE

Objective:To discuss the value of endoscopic diagnosis and treatment of Dieulafoy disease. Methods:Eighteen patients who had upper gastrointestinal bleeding from Dieulafoy disease were analysed retrospectively with regard to the clinical and endoscopic features, the method and outcome of therapy.Results:Of 18 patients, 13 were diagnosed in the first endoscopy examination ( 72.2 % ) ,5 needed 2 times or more examination .Fifteen cases were treated with endoscopy successfully. The initial therapy was successful in 12 cases ( 66.7 % ). Hemostasis was achieved with additional endoscopic therapy in 3 cases (16.6 % ). Surgical treatment were needed in 3 cases (16.6%).Conclusions: Endoscopic diagnosis and treatment should be regarded as the first choice for Dieulafoy disease.
