恩格菲联合博莱霉素胸腔灌注治疗恶性胸腔积液临床研究

目的探讨胸腔内联合灌注恩格菲和博莱霉素治疗恶性胸腔积液的临床疗效及安全性。方法经病理确诊的恶性胸腔积液97例,胸腔置管引流术排尽胸液后,随机分为2组,对照组(A组,47例)在胸腔内灌注博莱霉素1 mg/kg,治疗组(B组,50例)在胸腔内灌注恩格菲3 000 IU+博莱霉素1 mg/kg,观察疗效及毒副反应。结果A组完全缓解(CR)12例,部分缓解(PR)14例,总有效率55%;B组完全缓解(CR)23例,部分缓解(PR)20例,总有效率86%,两者比较有显著性差异(P<0.05)。2组不良反应主要为发热、胸痛、恶心,2组比较无显著性差异(P均>0.05)。不良反应经对症处理均可缓解。结论胸腔内联合灌注博莱霉素和恩格菲是一种有效、不良反应小、可耐受的疗法。

Clinical study of malignant pleural effusion treated by intrapleural perfusion of staphylococcal enterotoxin combined with bleomycin

Objective It is to approach the clinical curative effect and safety of staphylococcal enterotoxin combined with bleomycin regimens in the treatment of malignant pleural effusion.Methods 97 cases with malignant pleural effusion were randomly divided into 2 groups.Single-agent bleomycin 1mg/kg was infused into pleural cavity in group A;and staphylococcal enterotoxin 3 000 IU and bleomycin 1 mg/kg were infused into pleural cavity in group B.The curative effect and adverse reaction were observed.Results In group A,12 cases complete remission,14 cases partial remission, the total effective rate was 55%;in group B,23 cases complete remission,20 cases partial remission,the total effective rate was 86%,there was significant difference between the two groups(P<0.05).The major toxicities were fever, pleuritic pain,nausea,which could be relieved by reasonable treatment for the symptoms,there was no significant difference between the two groups(P>0.05).Conclusion The staphylococcal enterotoxin combined with bleomycin regimen is an effective and well-tolerated treatment in patients with malignant pleural effusion.
