2型糖尿病合并冠心病患者血浆同型半胱氨酸、脂蛋白(a)水平变化及其与胰岛素抵抗的关系

摘    要:目的:观察2型糖尿病(T2DM)合并冠心病(CHD)患者血浆总同型半胱氨酸(tHcy)、脂蛋白(a)Lp(a)]水平及其与胰岛素抵抗的关系。方法:将87例T2DM患者分为无并发CHD组(49例)和并发CHD组(38例),分别检测其血浆tHcy、Lp(a)、空腹血糖(FPG)和空腹胰岛素(FINS)浓度,计算胰岛素敏感性指数(ISI),并与对照组(68例)比较。结果:并发CHD组tHcy、Lp(a)浓度明显高于无并发CHD组(P<0.01),且并发CHD组tHcy、Lp(a)浓度与ISI均呈显著负相关(r=-0.342,P<0.05;r=-0.412,P<0.05),而无并发CHD组tHcy、Lp(a)浓度与ISI无显著相关(r=-0.211,P>0.05;r=-0.283,P>0.05)。结论:T2DM并发CHD患者血浆tHcy、Lp(a)明显升高,并与胰岛素抵抗密切相关。

Abstract:Objective:To explore correlation between plasma total homocysteine(tHcy),Lipoprotein(a)Lp(a)]levels and insulin resistance in diabetes CHD.Method:Determinating Parameters as tHcy、Lp(a)and FPG in the groups of normal control(n=68),diabetes non-CHD(n=49)and diabetes CHD(n=38),computing insulin sensitivity index(ISI).Results:The concentrations of tHcy and Lp(a)in diabetes CHD groups were significantly higher than no-CHD(P<0.01),The concentrations of tHcy and Lp(a)in diabetes CHD groups is negatively correlated significantly with ISI(r=-0.342,P<0.05;r=-0.412,P<0.05),and the concentrations of tHcy and Lp(a)in diabetes non-CHD groups weren't correlated(r=-0.211,P>0.05;r=-0.283,P>0.05).Conclusion:The concentrations of plasma tHcy and Lp(a)in diabetes CHD patientsn were significantly higher,and related with insulin resistance strongly.
