关节镜下肩峰成形术和肩峰下封闭治疗肩峰下撞击综合征的疗效对比研究

目的:观察比较关节镜下肩峰成形术和肩峰下封闭治疗肩峰下撞击综合征的临床疗效.方法:将66例确诊的肩峰下撞击综合征患者随机分为肩峰下封闭治疗组(对照组)及关节镜下肩峰成形术组(实验组),经随访采用VAS疼痛评分法和UCLA评分法,分别对两组进行疗效评定.结果:对照组随访平均时间分别为23.3个月,实验组平均随访21.8个月.VAS疼痛评分,对照组平均(2.6±1.8)分,实验组两组平均(1.1±1.3)分,两组差异有统计学意义(P =0.001).UCLA评分,对照组平均(29±2.3)分,实验组两组平均(33±1.2)分,两组差异有统计学意义(P=0.009).结论:关节镜下肩峰成形术治疗肩峰下撞击综合征将肩峰下进行有效的减压,提高了临床疗效,可作为治疗肩峰下撞击综合征更有效的方法.

Comparison of acromioplasty and subacromial block therapy for the treatment of subacromial impingement syndrome

Objective:To investigate the clinical effects of acromioplasty and subacromial block therapy in the treatment of subacromial impingement syndrome.Methods:From June 2006 to June 2010,66 patients sustained subacromial impingement syndrome were recruited into this study.These patients were divided into control group(subacromial block therapy) and treatment group(acromioplasty) in random.VAS and UCLA score were adopted for evaluation at the latest followed-up.Results:The mean followed-up period of control group was 23.3 months and that of treatment group was 21.8 months.VAS of control group and treatment group were 2.6±1.8 and 1.1±1.3 respectively.There was a statistically difference between the two groups(P=0.001).The UCLA score in control group was 29±2.3,and that in treatment group was 33±1.2.There was significant difference between the two groups(P=0.009).Conclusion:Arthroscopic acromioplasty in the treatment of subacromial impingement syndrome helps to gain improved clinical efficacy by the subacromial decompression,it is a safe and efficient therapy for subacromial impingement syndrome.
