以社区为基础的外来孕产妇管理模式初探及成效

目的:探索外来孕产妇管理模式,提高孕产妇保健覆盖面,体现保健公平性。方法:采用现场研究方法了解服务接受方与提供方对社区孕产期保健网络的需求及改进建议,在此基础上建立新的监测孕情及落实保健管理工作方案,再进行运行后的评估。结果:①建立社区为基础的孕情监测第四级网络,加强原有的妇幼三级网络的"网底"力量。监测孕情队伍的总人数为21716人,较以前增长了31.56%。②增加了孕情监测的渠道并提早了落实孕产期系统保健的时点,使本市户籍者的早孕建册率从92.71%上升到94.54%,外地户籍者从36.73%上升至46.65%。结论:以社区为基础的孕情监测网络的建立实施,有利于加强外来孕产妇保健管理,提高孕产期系统保健服务的覆盖面及提早落实孕产期保健服务,体现了"关口前移""重心下沉"及公平性与可及性。
