108例老年急性胆囊炎手术治疗临床分析

目的:为老年急性胆囊炎手术治疗提供科学依据。方法:以108例老年急性胆囊炎手术治疗患者为研究对象,通过对患者采用针对性治疗与观察,采用回顾性方法研究分析手术临床资料。结果:本组108例患者均于入院12～72h内手术,均获得急性胆囊炎手术治疗的成功,手术时间48～150min,平均时间(76.3±7.4)min,术中出血90～250mL,平均出血(142.6±10.1)mL,腹腔引流管在引流量低于12mL/d,并经复查B超无腹腔积液后拔除,术后住院天数3～12d,平均(10.1±3.4)d,术后出现切口感染2例,经加强换药后好转,保守治疗,充分引流后好转,无严重并发症出现。随访3～12个月,本组无死亡病例出现。结论:老年急性胆囊炎手术治疗是具有一定困难和风险的,只有熟练掌握胆囊腹腔镜手术治疗方法和相关技能,才能取得较好的手术效果。

Clintcal analysis of 108 cases of elderly patients with acute cholecysitis

Objective:To further make elderly patient with acute laparoscopic gallbladder surgery to provide a scientific basis.Methods:108 cases of laparoscopic gallbladder surgery,the acute of patients in this study,through the use of targeted therapies for patients with observation,a retrospective analysis of surgical methods of clinical data.Results:The 108 patients were admitted to hospital within 72 hours of surgery,and both were successful laparoscopic cholecystectomy,operative time 48~150 min,mean(76.3±7.4) min,blood lo-ss 90~250mL,average(142.6±10.1) mL,peritoneal drainage tube drainage was less than 12mL/d,and has been inspected and B-free after removal of ascites,postoperative hospital stay 3~12d,average(10.1±3.4) d,postoperative wound infection in 2 cases,the enhanced dressing improved after conservative treatment,adequate drainage improvement,no serious complications were followed up for 3 to 12 months,no deaths in this group appear.Conclusion:The elderly patients with acute cholecystitis operation treatment is a difficult and risks,and only master the complexity of laparoscopic surgical treatment of gallbladder and related skills in order to achieve better surgical results.
