人工耳蜗植入术53例报告

目的总结人工耳蜗植入手术的经验。方法 2004年3月~2009年1月施行53例人工耳蜗植入术,对6例合并分泌性中耳炎的人工耳蜗植入采用先彻底清除病变再植入人工耳蜗;其余的人工耳蜗植入采用乳突切开充分暴露面神经隐窝而非彻底轮廓化乳突。结果 53例人工耳蜗植入均一次成功,术中神经反应遥测（neural response telemetry,NRT）正常,术后植入电极X线摄片正常。53例术后随访2个月~5年,平均2.8年,纯音听力水平52~33dB SPL,平均42.4dB SPL,无并发症发生。结论人工耳蜗植入手术的成功依赖于手术前详细的影像学资料,手术过程的程序化和细致操作,以及良好的器械条件。

Cochlear Implantation: Analysis of 53 Cases

Objective To summarize our experiences in cochlear implantation.Methods Cochlear implantation was performed on 53 patients with severe hearing loss or total deaf from March 2004 to January 2009.For 6 patients who had secretory otitis media,cochlear implantation was carried out after complete eradication of the lesion.In the rest of the patients,cochlear implantation was done by minimally invasive methods.Results The implantation was completed successfully in all the patients without complications.Neural response telemetry showed normal results during the operation,and postoperative X-ray revealed that the implanted electronic device was in a good condition in all the patients.Follow-up was achieved in all the cases for 2 months to 5 years with a mean of 2.8 years,during which pure tone audiometry showed 52-33 dB SPL（mean,42.4 dB SPL）.ConclusionPreoperative imaging,careful operation,and high-quality operative instruments are keys to the success of cochlear implantation.
