小脑幕枕骨附着处位置的磁共振成像研究

摘    要:目的:探讨小脑幕枕内附着处位置正常范围及发育规律。材料与方法:测量317例正常人和42例Chiari畸形及可疑患者小脑幕枕内附着点高距及斜距。正常人按性别、年龄分16组,每组20例(≥70岁女性组17例);应用10T超导MR扫描机;用t检验作统计分析。结果:其正常值范围高距男性为(289±46~335±45)mm,女性为(304±40~330±52)mm;斜距男性为(383±43~418±41)mm,女性为(367±50~398±30)mm;部分组间有显著年龄及性别差异,且其均显著大于Chiari畸形及可疑患者的。结论:小脑幕枕内附着处位置有一定变化规律,其位置过低可致Chiari畸形。

Abstract:Purpose: To measure the position of occipital attached point of cerebellum tentorium (POAT) with MR imaging, in order to determine the pathologicoanatomy and the normal range of Chinese parameters. Materials and Methods: The vertical and oblique distance from the occipital attached point of cerebellum tentorium to the level of foramen occipital magnum on mid-or paramidsagittal SE T1-weighted image (VDOAT and ODOAT) in 317 normal Chinese aged from birth to over 70, and in 42 patients with Chiari malformations. MRI data were obtained by 10T MR scanner, and t-Test was used as statistically significance analysis. Results: The normal VDOAT was range from 289mm4.6mm to 33.5mm4.5mm for the male and range from 30.4mm4.0mm to 33.0mm5.2mm for the female. The normal ODOAT was range from 38.3mm4.3mm to 41.8mm4.1mm for the male and range from 36.7mm5.0mm to 39.8mm3.0mm for the female, with statistical difference in partial different groups of sex or age, and they are all larger than that of Chiari malformations (P<0.05). Conclusion: The POAT has regular rule of change in all life, and it may cause Chiari malformations as it is too low.
