颈动脉内-中膜厚度与冠心病的病变程度及其危险因素的相关性研究

摘    要:目的探讨超声检测颈动脉内-中膜厚度在冠心病病变程度及其危险因素的相关性研究中的意义.方法随机抽取在我院接受冠状动脉造影检查的患者79例,同时应用超声技术测量双侧颈总动脉、分叉处及颈内动脉后壁内-中膜厚度;应用全自动生化分析仪测定各研究对象血糖、血脂等指标.采用多元线性回归分析颈动脉内-中膜厚度与冠心病病变程度及其危险因素的关系.结果颈动脉内-中膜厚度与冠状动脉造影的病变支数呈正相关,与冠心病的危险因素如年龄、低密度脂蛋白胆固醇、血压、血糖、男性别呈正相关,而与高密度脂蛋白胆固醇水平增高呈负相关.结论超声检测颈动脉内-中膜厚度可早期反映冠状动脉粥样硬化,前瞻性地分析冠状动脉病变程度,且对于冠心病危险因素水平予以评估,可作为监测冠状动脉疾病发生、发展及治疗效果的一项有临床意义的指标.

Abstract:Objective To determine the relationship between carotid arterial intima-media thickness(IMT),angiography findings and coronary artery atherosclerosis risk factors accessed by noninvasive high-frequency ultrasound.Methods Seventy-nine subjects aged from 38 to 82 years underwent coronary angiography and were investigated for coronary risk factors.Their lower-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),blood pressure,glucose were measured.Meanwhile the maximum IMT of the distal common carotid,carotid bifurcation and proximal internal carotid,both right and left were measured by ultrasound.Results The thickness of the carotid arterial intima-media increased with the degree of coronary atherosclerosis disease and increasing age,low-density lipoprotein,blood pressure,glucose and gender.Conclusion Carotid ultrasound is a safe noninvasive and relatively inexpensive means of accessing clinical and sub-clinical atherosclerosis.Evidence substantiates that IMT correlates with the presence of coronary atherosclerosis and coronary artery disease risk factors.Moreover IMT can be used as a therapeutic endpoint.
