摘    要:目的:探讨蕲蛇酶治疗对短暂性脑缺血发作(TIA)患者凝血和纤溶指标的影响。方法:80例TIA患者随机分为蕲蛇酶处理组和对照组,所有病例均接受标准的综合治疗,并在入院时检测凝血酶原时间(PT)、活化部分凝血激酶时间(aPTT)、纤维蛋白原(Fg)和D二聚体(DD)水平,然后在入院后3、5、7、10d时重复测定上述指标。随访时间为3个月。主要转归指标为急性脑梗死和死亡。结果:2组患者基线PT和aPTT水平无明显差异(P>0.05)。治疗组Fg水平在第5天后迅速下降,DD浓度在第3天后开始逐步下降;对照组Fg和DD水平至第10天时仍然保持在第3天和第5天时的高峰水平。治疗组短期转归明显优于对照组(P<0.05)。结论:蕲蛇酶治疗能有效降低Fg和DD水平,改善TIA患者的近期转归。

Abstract:Objective: To investigate the effect of acutobin on coagulation and fibrinolytic systems in patients with transient ischemic attack (TIA).Methods: Eighty patients with TIA were randomly allocated into acutobin group and control group. They all received standard treatment. Their levels of prothrombin time (PT), active partial thromboplastin times (aPTT), fribrinogen (Fg), D-dimer (DD) were measured at admission. The above indices were remeasured at day 3, 5, 7 and 10, respectively. The follow up time was 90 days. The primary outcome endpoint was acute cerebral infarction and death. Results: There was no significant difference at baseline in both groups. The Fg level decreased rapidly 5 days later, while the DD level began to decrease 3 days later in the treatment group. The levels of Fg and DD at day 10 still remained the highest levels at day 3 and 5. The short-term outcome in the treatment group was significantly better than that of the control group (P>0.05). Conclusions: Acutobin therapy can effectively decrease the levels of Fg and DD, and improve the short-term outcome in patients with TIA.
