子宫内膜间质肉瘤14例临床分析

目的:探讨子宫内膜间质肉瘤的诊断、治疗及预后。方法:回顾性分析我院1999年6月～2006年6月收治的14例子宫内膜间质肉瘤的诊治方法和结果。结果:确诊主要依靠病理结果,术前病理检查7例,其中阳性5例,术中快速病理检查确诊6例,3例术前误诊者术后通过石蜡病理明确诊断。7例采用全子宫及双附件切除术,4例采用广泛性子宫及双附件切除加盆腔淋巴结清扫术,3例二期手术。全部病例均接受化疗或放疗。结论:诊断性刮宫或宫颈赘生物活检、术中快速病理检查可降低误诊率;子宫内膜间质肉瘤的恶性度与预后关系密切,低度恶性者切除范围足够大,手术前后辅助化疗或放疗可减少复发和提高存活率。

Clinicopathologic and Prognosis Analysis of 14 cases Endometrial Stromal Sarcomas

Objective:To study diagnosis,prognosis and management of endometrial stromal sarcoma(ESS).Methods:The medical data of the 14 cases with ESS treated between June 1999-June 2006 in our hospital was analyzed retrospectively.Results:Final diagnosis mainly depended on pathological results.Before operation,7 cases were checked with pathological method,with 5 cases being positive.The diagnosis of 6 cases were confirmed by frozen section during operation and 3 by paraffin wax after operation.7 patients had hysterectomy and bilateral appendectomy,4 patients had radical hysterectomy and 3 patients had a second operation.All patients took chemotherapy and radiotherapy after operations.Conclusion:Diagnostic curettage and frozen section during the operation can reduce misdiagnosis rate.The malignant degree of ESS is closely related to prognosis.It is advisable to perform wider resections assisted by radiotherapy or chemotherapy to reduce recurrent and increase survival rate.
