静脉注射胺碘酮转复阵发性心房颤动的临床观察

目的：观察静脉注射胺碘酮转复阵发性心房颤动（PAF）的疗效和安全性。方法：80例发作1～24h的PAF患者，随机分为两组。胺碘酮组（40例）：胺碘酮150mg静脉注射，继以1mg／min静脉泵入．维持6h,去乙酰毛花苷组（40例）：去乙酰毛花苷0．4mg静脉注射，2h后追加0．2mg。观察PAF转复情况，心率、QTe是期变化及副作用。结果：胺碘酮组复律29例（72％），去乙酰毛花苷组复律18例（45％），P〈0．05。复律时间分别为（215±58）min。（253±39）min．P〈0．05。未转复者心率分别下降26％和11％。P〈0．05。QTc间期两组复律前后比较差异无显著性。两组均无严重副作用。结论：静脉注射胺碘酮转复PAF有效且安全。

Effect of amiodarone intravenous iniection on conversion of paroxysmal atrial fibrillation

Objective： To study the clinical effect and safety of amiodarone intravenous injection on conversion of paroxysmal atrial fibrillation （PAF）. Methods： Eighty patients with PAF for 1-24 h were divided into two groups. Forty cases in trail group received 150mg amiodarone intravenous injection, followed by lmg/min infusion for 6 h; forty cases in control group received 0. 4mg deslanoside intravenous injection, followed by 0. 2mg deslanoside intravenous injection 2 h later. The time of sinus rhythm converted when drug was used was recorded. The heart rate, QTc interval and side effects of the drugs were observed during study. Results.. The rate of conversion in trail group was 720%; while the rate of conversion in control group was 45%, P〈0.05. Conversion time in trail group was （215±58） rain, while that in control group was （253± 39） min, P〈0. 05. In patients whose cardioversion failed of two groups the heart rate decreased 26% and 11% respectively, P〈0. 05. QTc interval has no statistical difference in two groups before and after eardioversion. Both two groups had not severe side effects. Conclusion： Amiodarone intravenous injection is efficient and safe in conversion of paroxysmal atrial fibrillation.
