摘    要:目的探讨草分枝杆菌对难治性结核性脑膜炎患者外周血中T淋巴细胞亚群的影响。方法选取2005年4月至2007年9月住院的未经抗结核治疗的难治性结核性脑膜炎患者84例,随机均分为两组。治疗组采用草分枝杆菌+HRZE+氟美松+鞘内注药方案.对照组仅用HRZE+氟关松+鞘内注药方案治疗,用药4周。结果治疗组临床疗效明显优于对照组;与治疗组比较,CD3^+及CD4^+细胞明显增加.CD8^+细胞降低,CD4^+/CD8^+值显著升高(P〈0.01)。结论草分枝杆菌对难治性结脑患者具有细胞免疫调节作用,疗效肯定,副作用小.是理想的抗结核辅助用药。

Abstract:Objective To investigate the effect of utilins on peripheral T-lymphocyte subsets in refractory tuberculous meningitis. Methods 84 inpatients of refractory tuberculous meningitis without antituberculosis treatment in April 2005 to September 2007, 25 females and 59 males, aged 15-78 years, were selected and randomly divided into two groups. The treatment group was given utilins + HRZE + dexam- ethasone + intrathecal injection program, the control group was only given HRZE + dexamethasone + intratheeal injection program. The ther- apeutic courses all were 4 weeks. Results The effect in the treatment group was better than that in the control group. Compared with the treatment group, CD3^+ and CD4^+ markedly increased and CD8^+ decreased, the value of CD4^+/CD8^+ obviously increased(P 〈 0. 01). Conclusion Utllins treating refractory tuberculous meningitis has a role in cellular immune regulation with certain curative effect and less side effects. It is an ideal adjuvant anti- TB drug.
