儿童颅咽管瘤全切术后并发症的分析

目的探讨儿童颅咽管瘤全切术后并发症特点及治疗措施。方法选取2015年1月至2018年7月行颅咽管瘤全切术的78例儿童(观察组)及78例成人(对照组)进行回顾性分析,对比两组患者术后并发症及生活质量情况,并探讨治疗措施。结果观察组术后并发症有:电解质紊乱72例(92.31%)、尿崩症67例(85.90%)、垂体功能异常45例(57.69%)、高热12例(15.38%)、意识障碍9例(11.54%)、癫痫发作7例(8.97%);对照组术后并发症有:电解质紊乱68例(87.18%)、尿崩症58例(74.36%)、垂体功能异常38例(48.72%)、高热8例(10.26%)、意识障碍7例(8.97%)、癫痫发作6例(7.69%),对比两组患者各并发症发生率差异均无统计学意义(P>0.05)。术后观察组患儿:生活自理级52例、生活半自理级16例,生活帮助级10例;对照组患儿:生活自理级56例、生活半自理级14例、生活帮助级8例,对比两组患者KPS评分分级分布差异无统计学意义(P>0.05)。结论儿童颅咽管瘤术后并发症发生率高且病情复杂,但其发生率相较于成人无显著差异,术后加强监测,积极对症处理能改善患儿生活质量。

Analysis of Complications After Total Resection Ofcraniopharyngioma in Children

Objective to investigate the characteristics and treatment of complications after total resection of craniopharyngioma in children. Methods Retrospective analysis was performed on 78 children( observation group) and 78 adults( control group) who underwent total craniopharyngioma resection from January 2015 to July 2018. Postoperative complications and quality of life of the two groups were compared,and treatment measures were discussed. Results Postoperative complications in the observation group included: 72 cases of electrolyte disturbance( 92.31%),67 cases of diabetes diabetes mellitus( 85.90%),45 cases of abnormal pituitary function( 57.69%),12 cases of high fever( 15.38%),9 cases of consciousness disturbance( 11.54%),and 7 cases of epilepsy( 8.97%). Postoperative complications in the control group included: 68 cases of electrolyte disturbance( 87.18%),58 cases of diabetes diabetes mellitus( 74.36%),38 cases of pituitary dysfunction( 48.72%),8 cases of high fever( 10.26%),7 cases of consciousness disturbance( 8.97%),and 6 cases of epilepsy( 7.69%). The difference in the incidence of complications between the two groups was not statistically significant( P>0.05). Postoperative observation group: 52 children with self-care level,16 with semi-self-care level,and 10 with life help level. In the control group,there were 56 cases with self-care grade,14 cases with semi-self-care grade and 8 cases with life help grade. There was no statistically significant difference in KPS grading distribution between the two groups( P> 0.05). Conclusion The incidence of postoperative complications of craniopharyngioma in children is high and complicated,but its incidence is not significantly different from that of adults.Postoperative monitoring and active symptomatic treatment can improve the quality of life of children.
