针刺足三里及丰隆治疗顽固性心力衰竭30例临床观察

目的:观察针刺足三里、丰隆穴联合西药治疗顽固性心力衰竭(下称心衰)的临床疗效。方法:选取符合标准的90例顽固性心衰患者,采用随机数字表法分为针刺组、揿针组及对照组各30例,三组患者均给予基础强心利尿措施治疗,针刺组在常规西药基础上联合普通针刺足三里、丰隆,揿针组在常规西药基础上联合微型皮内针治疗,治疗4周后观察三组患者左室射血分数(LVEF)、血浆氨基末端脑钠肽前体(NT-proBNP)及明尼苏达心衰生活质量量表(MLHFQ)评分变化。结果:治疗后,三组患者的LVEF均增高,血浆NT-proBNP水平、MLHFQ评分均降低(P<0.01);针刺组的LVEF高于揿针组及对照组(P<0.01),血浆NT-proBNP、MLHFQ评分均低于揿针组及对照组(P<0.01);揿针组与对照组相比,LVEF、血浆NT-proBNP、MLHFQ评分差异均无统计学意义(P>0.05)。结论:针刺足三里、丰隆能显著改善心脏功能,提高患者的生活质量。

Clinical Observation on Acupuncture at Zusanli and Fenglong in Treating 30 Patients with Refractory Heart Failure

Objective:To explore the therapeutic effect of acupuncture at Zusanli and Fenglong acupoints combined with western medicine on refractory heart failure.Methods:Total 90 patients with refractory heart failure who met the standard were selected and divided into acupuncture group,press needle group and control groups by random number table method,30 patients in each group.The three groups of patients were given basic cardiac diuretic measures,the acupuncture group combined with conventional acupuncture at Zusanli and Fenglong on the basis of conventional western medicine,and the press eedle group combined with mini-intracutaneous acupuncture on the basis of conventional western medicine,all treated for 4 weeks.The left ventricular ejection fraction(LVEF),plasma amino-terminal brain natriuretic peptide precursor(NT-proBNP)and Minnesota Heart Failure Quality of Life Scale(MLHFQ)scores were compared among the three groups.Results:After treatment,the LVEF measurement values of the three groups of patients increased,and the plasma NT-proBNP measurement value and MLHFQ score decreased(P<0.01);the LVEF of the acupuncture group was higher than that of the press needle group and the control group(P<0.01).The plasma NT-proBNP and MLHFQ scores of the acupuncture group were lower than those of the press needle group and the control group(P<0.01);compared with the control group,the LVEF,NT-proBNP and MLHFQ scores of the press needle group were not statistically significant(P>0.05).Conclusions:Acupuncture at zusanli and fenglong acupoints can significantly improve the cardiac function and the life quality of patients with refractory heart failure.
