基于数据挖掘对王树庆辨治慢性再生障碍性贫血的临床经验总结

目的:系统总结分析王树庆教授治疗慢性再生障碍性贫血(再障)的用药规律,通过对其临床用药数据进行挖掘,归纳其治疗慢性再障的临床经验。方法:搜集2014年11月至2017年12月门诊治疗的34例慢性再障病例,将患者临床证候及用药等资料进行统计整理,使用SPSS 19.0对病例中的中药进行频数统计分析,对频数较高的前36味中药进行系统聚类分析。结果:34例患者140诊次共用中药153味,分别属于9种功效不同的类别,累计用药频次2228次,使用频次最多的是熟地黄,其次为山萸肉、当归、泽泻、黄芪等。对36味高频药物进行聚类分析,得出6类不同的用药组合。结论:依据用药组合结合临床实践,总结王树庆治疗慢性再障的经验。王树庆辨治慢性再障以补肾、活髓、通络为根本治疗大法,以愈障汤为基础方,用药灵活,疗效肯定。

Summary of Clinical Experience of WANG Shuqing in Treating Chronic Aplastic Anemia Based on Data Mining

Objective:To summarize and analyze Professor WANG Shuqing’s medication regularity in the treatment of chronic aplastic anemia,and to summarize his clinical experience in the treatment of chronic aplastic anemia by mining his clinical medication data. Methods:A total of 34 cases of chronic aplastic anemia treated in outpatient clinics from November 2014 to December 2017 were collected and the clinical syndrome and medication data of the patients were statistically sorted out. The frequency of Chinese medicinals in the cases was statistically analyzed by SPSS 19.0,and the 36 Chinese medicinals with high frequency were systematically clustered. Results:In the 140 consultations of the 34 patients,there were 153 Chinese medicinals,belonging to 9 different efficacy categories,and the cumulative frequency of medication was 2228 times. The most frequently used medicinals was Shudihuang(Rehmanniae Radix Praeparata),which was followed by Shanyurou(Corni Fructus),Danggui(Angelicae Sinensis Radix),Zexie(Alismatis Rhizoma),Huangqi(Astragali Radix),etc. The cluster analysis of 36 high-frequency medicinals was carried out,and 6 different combinations of medicinals were obtained. Conclusions:According to the combination of medication and clinical practice,the experience of Professor WANG Shuqing in treating chronic aplastic anemia was summarized. In the treatment of chronic aplastic anemia,WANG Shuqing takes tonifying the kidney,activating the marrow and unblocking the collaterals as the basic methods,and by using Yuzhang Decoction(愈障汤) as the basic prescription,the medication was flexible and the effect was positive.
