周围型肺腺癌及淋巴结增大SCT征象与肿瘤微脉管形成关系的探讨

摘    要:目的研究周围型肺腺癌及淋巴结增大的螺旋CT征象与肿瘤微脉管(淋巴管、微血管)形成的相关性。方法搜集40例经手术病理证实的肺腺癌,术前均行CT平扫及增强扫描,对标本行SP免疫组化染色,计数其淋巴管密度(LM-VD)及微血管密度(MVD),分析肺腺癌及淋巴结转移某些SCT表现与LMVD及MVD之间的关系。结果LMVD、MVD值与淋巴结转移密切相关,肺癌强化幅度、瘤体大小、淋巴结增大、棘突征、毛刺征、分叶征、血管集束征、淋巴结大小、周围脂肪间隙情况等CT表现与LMVD和(或)MVD有相关性。结论肺腺癌的棘突征、毛刺征、分叶征、强化幅度、瘤体大小及淋巴结增大等CT征象与肿瘤的微脉管生成有关,可以一定程度预测肺癌的侵袭及转移。

Abstract:Objective To investigate the relationship between spiral computed tomography (SCT) features with tumor lymphangiogenesis, angiogenesis in peripheral lung adenocarainomas and lymph nodes. Methods Forty patients with surgically and pathologically proved lung adenocarainomas and examined with plain and enhanced SCT scanning were included in the study. SP immunohistochemical technique was experimented in all pathologic specimens. According the immunohistochemical results of VEGFR-3 and CD34 to get the lymphatic microvessel density and microvessel density. The spiral CT features were compared and analyzed with immunohistochemical results of peripheral lung adenocarainomas. Results The correlation was shown respectively between lymph node metastasis and the value of LMVD and MVD. The CT feactures (CT enhancement degree, the diameter of tumor, lymph nodes growth, spiculated sign, spiculation, lobulation, blood vessel convergency, the diameter of lymph node, lymph node rim of fat, et al.) were also relevant to the mean value of MVD and LMVD. Conclusion Some SCT appearances (CT enhancement degree, the diameter of tumor, lymph nodes growth, spiculated sign, spiculation, lobulation, the diameter of lymph node, et al.) were closely correlated with MVD, LMVD in lung adenocarainomas, which could be a noninvasive method in predicting aggressiveness and metastasis.
