空心钉与钢板内固定治疗老年后踝骨折的疗效

目的比较空心钉与钢板内固定治疗老年后踝骨折的疗效。方法将87例老年后踝骨折患者按内固定方式分为空心钉组(39例)及钢板组(48例)。术后3周比较两组并发症情况。术后12个月采用AOFAS评分、疼痛VAS评分和足部对线评分评价疗效。末次随访比较两组患者满意率。结果患者均获得随访,时间12~18(15.12±3.72)个月。骨折均愈合。术后3周并发症发生例数空心钉组较钢板组少(P<0.05)。术后12个月AOFAS评分、VAS评分空心钉组优于钢板组(P<0.05),但足部对线评分两组比较差异无统计学意义(P>0.05)。末次随访时患者满意率两组比较差异无统计学意义(P>0.05)。结论老年后踝骨折可优先选择空心钉内固定。

The effect of cannulated screw and plate internal fixation in the treatment of elderly patients with posterior malleolus fracture

Objective To compare the effect of internal fixation with cannulated screw and plate on posterior malleolus fracture in the elderly.Methods The 87 elderly patients with posterior malleolus fractures were divided into the cannulated screw group(39 cases)and the plate group(48 cases)according to the surgical fixation method.The complications of the two groups were compared at 3 weeks after surgery.AOFAS score,pain VAS and foot alignment score were used to evaluate the efficacy at 12 months after the operation.The satisfaction rates of the two groups were compared at the last follow-up.Results All patients were followed up for 12~18(15.12±3.72)months.All fractures were healing.The number of complications in the cannulated screw group was less than that in the plate group at 3 weeks postoperation(P<0.05).AOFAS score and pain VAS of the cannulated screw group were better than those of the plate group at 12 months after surgery(P<0.05),but there was no significant difference in foot alignment between the two groups(P>0.05).At the last follow-up,there was no statistical difference in patient′s satisfaction rate between the two groups(P>0.05).Conclusions Cannulated screw is preferred for elderly patients with posterior malleolus fractures.
