溃疡性结肠炎患者肠粘膜固有层淋巴细胞表达IL—6的研究

目的：研究溃疡性结肠炎（UC）粘膜固有层单核细胞（LPMC）白细胞介素－6（IL－6）的表达和分泌，以及与疾病活动性的关系。方法：用ELISA法测定32例UC患者及40例正常对照者血清和细胞培养液中IL－6的浓度，用逆转录聚合酶链反应法测定IL－6 mRNA的表达，结果：UC患者血清中IL－6的浓度无明显升高，但外周血单核细胞（PBMC）在PMA和抗CD3的刺激下表达IL－6 mRNA及分泌IL－6的水平明显增加。UC病变部位的LPMC表达大量的IL－6 mRNA并自发分泌大量的IL－6蛋白，而且IL－6浓度与患者外周血清中的C－反应蛋白水平，血沉均呈显著正相关（γ＝0．69，γ＝0．61，P均＜0．01）。在LPMC培养液中加入IL－17后LPMC分泌IL－6明显增加，加入抗IL－17的单克隆抗体后，IL－6的分泌则显著被抑制，结论：UC患者病变部位的LPMC表达和分泌IL－6明显增加，并且与疾病的活性呈正相关，IL－7在调节LPMC产生IL－6过程中发挥重要作用。

Expression of interleukin-6 by lamina propria mononuclear cells in the patient with ulcerative colitis

Objective:To investigate the expression and secretion of int er leukin-6(IL-6)by lamina propria mononuclear cells(LPMC)in the patient with ulc erative colitis(UC).Methods:IL-6 was measured using an ELISA te chnique,and IL-6 mRNA was detected by RT-PCR in 32 patients with UC and 4 0 control subjects.In addition,the contribution of exogenous IL-17 and a nti-IL-17 monoclonal antibody(MoAb)to IL-6 production by LPMC was studied.Results:As compared to controls, serum concentrations of IL-6 from UC patients were higher,but with no statistic difference.However,the expression of IL-6 mRNA and secretion of IL-6 by the peripheral blood mononuclear cells in the UC patients were higher than that in the normal controls in the presence of stimuli(both P<0.01).LPMC from the UC involved colons expressed large amo unt of IL-6 mRNA and spontaneously produced higher level of IL-6 protein c ompared with LPMC from the uninvolved colons did,which significantly correlated with the serum CRP levels and ESR,respectively.Addition of exogenous IL-17 to LPMC from involved colons resulted in increase of IL-6 production in vitro.Howe ver,addition of anti-IL-17 MoAb resulted in decrease of IL-6 secretion signif icantly.Conclusion:LPMC from UC involved colons expressed high l evels of IL-6 mRNA and produced higher levels of IL-6 protein, which was corre lated with disease activity.IL-17 plays an important modulating role in the IL -6 secretion by LPMC.
