~(99m)Tc-MIBI心肌断层显像对心脏病的诊断应用

摘    要:为了观察99mTcMIBI正常人心肌显像特点及其在诊断冠心病、病毒性心肌炎、心肌病等方面的临床应用价值,正常对照42例,冠心病141例,病毒性心肌炎36例,心肌病23例,分别进行静息相99mTcMIBI心肌断层显像,部分患者加作运动相或硝酸甘油介入99mTcMIBI心肌断层显像。于水平长轴、垂直长轴、短轴及靶心图上将左室心肌分为心尖、前壁、前侧壁、侧壁、后侧壁、下后壁、前间壁、间壁、后间壁9个节段,观察其放射性分布情况。结果:正常对照组,左室心肌放射性分布不均匀,心尖、下后壁、间壁可见相对稀疏区;99mTcMIBI心肌显像诊断冠心病、病毒性心肌炎和心肌病的灵敏度分别为92.2%,86.1%和100%。结果提示:99mTcMIBI心肌断层显像,不仅对冠心病的诊断有较大的价值,而且对病毒性心肌炎、心肌病的诊断亦有一定的意义。

Abstract:In order to observe the characteristic of myocardial imaging with 99m Tc MIBI in normal persons and evaluate it in diagnosing coronary artery diseases (CAD),virus myocarditis(VM) and cardiomyopath (CM),42 cases of normal controls,141 cases of CHD,36 cases of VM and 23 cases of CM accepted rest 99m Tc MIBI myocardial SPECT.Some of them underwent an exercise stress test or nitroglycerine administration.The results indicated that the radioactive distributions of left ventricular myocardium in the control group were not ideally distributed.The apex,septal,inferion and posterior wall may appear less active.The sensitivity of 92.2%,86.1% and 100% were found in the CAD,VM and CM respectively.It was demonstrated that 99m Tc MIBI had significant diagnostic value not only in CAD but also in VM and CM.
