成人肱骨远端骨折的手术治疗分析

目的探讨成人肱骨远端骨折(含髁间骨折)的手术内固定方法和疗效。方法 2006年1月—2009年6月手术治疗成人肱骨远端骨折患者50例,其中陈旧性骨折3例,开放性骨折10例。AO/ASIF分型:A型3例,B型7例,C型40例。按照不同的骨折类型采用相应的内固定并进行功能锻炼。定期随访并手术后1、3、6个月拍片,了解骨折愈合和关节功能恢复情况。结果 50例均获得随访,随访时间6～24个月,平均13个月,50例均骨折愈合,肘关节功能恢复满意。结论对成人肱骨远端骨折采用手术方法 ,术中充分显露,骨折关节面解剖复位,在髁上部利用钢板,螺钉,克氏针合理运用牢靠固定,可以获得满意的治疗效果。

Surgical operation of adults' distal humeral fractures

Objective To study the internal fixation for adults' distal humeral fractures(including inter-condylar fractures)and its outcome.Methods A total of 50 adult patients with distal humeral fractures(including three with dated fractures and ten with open fractures)were performed surgical operation between Jan.2006 and Jun 2009.According to AO/ASIF classification,the factures were classified as type A(n=3),B(n=7)and C(n=40).Internal fixation was applied to the patients,who did functional exercises after operation,on the basis of classification.The healing of the fractures and the function the joints were examined via follow-up and X-ray films 1,3 and 6 months after operation.Results 50 patients,who were followed up with a time of 6-24 months(mean 13 months),were healed of the fractures with functionally satisfactory recovery of elbow joints.Conclusion Correct surgical operation,i.e.internal fixation with plate,screws,Kirschner wires,etc,is an effective therapeutic approach to distal humeral fractures in adults.
