超声心动图评价左心房成形术治疗风心病巨大左心房的疗效

目的：利用超声心动图评价左心房成形术对心脏形态和心脏功能的影响。方法：48例在浅低温体外循环下施行二尖瓣置换加左房成形术的风心病二尖瓣病变合并巨大左心房患者分为两组，A组：单纯二尖瓣狭窄（MS）26例；B组：二尖瓣狭窄并二尖瓣关闭不全（MSMI）22例。于手术前和手术后两周接受超声心动图检查。结果：两组心脏左心房收缩末期径（LASD）术后均明显下降（P＜0．01）；A组左心室心肌重量指数（LVMI）于手术后明显增加（P＜0．01），而B组心肌重量指数（LVMI）轻度下降（P＜0．05）；两组患者手术后心功能（EF）均有提高（P＜0．05）。结论：二尖瓣置换加左心房折叠成形术对术前左心功能不全的风心病二尖瓣病变合并巨大左心房患者的心脏缩小和心功能的恢复有较大帮助。

Evaluae The Left Atrium Plastying Operation To Treat Rheumatic Heart Disease With Gaint Left Atrium

Objective Left ventricle function and heart configuration were studied by echcardiongraphy before and after MVR add the plastying GLA operation.Methods 48 MVHD with GLA cases were divided into 2 groups.All cases underwent mitral valve replacement and plastied gaint left atrium on beating heart intracardiac operation.They all werw checked by echocardingraphy before and after operation in 2 weeks.Group A:26 cases MS+GLA,Group B:22 cases MSMI+GLA.Results The LAEDD obviously descwended after operationin 2 groups( P <0.01).The LVMI obviously increased after operation in group A( P <0.01) and gently declined in group B( P <0.05).The EF improved after operation 2 groups( P <0.05).Conclusion MVR add GLA plastying operation is a good surgical treatment for the MVHD with GLA cases.
