76例气管支气管结核临床,X线及纤维支气管镜下特征分析

为提高对气管支气管结核的认识，对７６例气管支气管结核患者的临床，胸部Ｘ线及纤维支气管镜（纤支镜）下有表现进行了分析总结。结果：男女之比１：１．３，平均年龄４５．９岁，临床上以咳嗽为主，占７１．１％，其中７７．８％的患者表现为刺激性干咳，发热，咯血分别占２３．７％和１９．７％，胸片表现斑片浸润影者３２．８％，肺门增大或块影及叶段肺不张者分别为１５．８％和１３．４％，６．６％的患者胸片阴性。纤支镜下表

ENDOBRONCHIAL TUBERCULOSIS: CLINICAL,ROENTGENOGRAPHIC AND FIBEROPTIC BRONCHOSCOPIC FEATURES IN 76 CASES

In order to heighten our awareness of endobronchial tuberculosis, the clinical, roentgenot;raphic and fiberopticbronchoscopic features of endobronchial tuberculosis in 76 cases were reviewed. The male to female ratio was 1: 1. 3,and the mean age was 45. 9 years. Cough was the chief complaint (71. 1 % ), next were fever (23. 7% ) and hemoptysis (19. 7 % ). Parenchyrnal infiltration was the most common roentgenographic finding of the chest (32. 8 % ), nextwere hilar enlargement or mass (15. 8 % ), and lobar or segment atelectasis (13. 4 % ). Normal roentgenograms wereobserved in 6. 6% of the patients. Bronchoscopically, granulomatous lesions, mucosal inflammatory infiltration , ulceration or caseous necrosis and cicatricial stenosis were observed in 47. 4%, 39. 5 %, 23. 7 % and 14. 5% of the patients, respectively. Upper lobar apical segment endobronchial tuberculosis was found in 11. 8% of the patients, upper lobar and middle (tongue) in 46. 1 % of the patients, and lowr lobar endobronchial tuberculosis in 63. 2 % of thepatients. Bronchootenosis was found in 59. 2% of the patients. Sputum srneax for acid-fast bacilli via bronchaywas positive in 7. 8% of the patients. Endobronchial tuberculosis has some identifying features, but mtimes apPears very similar to lung cancer. It is necessary that suspect petients be examined with fiberoptic bronchacoy,
