肝癌栓塞治疗前后彩色多普勒超声与血管造影对照观察疗效的研究

目的 探讨彩色多普勒超声（ＣＤＦＩ）评价肝癌栓塞疗效及诊断肿瘤复发的价值。方法 对３６例肝癌患者栓塞前后ＣＤＦＩ、血管造影和ＣＴ做对照分析。结果 ３６例中３４例（９４．４％）栓塞前ＣＤＦＩ显示有血流,分为４组图形（１）肿瘤周边搏动性血流;（２）肿瘤中心搏动性血流;（３）肿瘤周边连续性血流;（４）肿瘤中央的连续性血流。血管造影与ＣＤＦＩ对照,前两组发现相应血管,其余两组未见相应血管。栓塞二周后,３６

Comparison of Color Doppler Sonography with Angiography in Detecting the therapeutic Effect of Hepatocellular Carcinoma before and after Transcatheter Arterial Embolization

ObjectiveToevaluate of color Doppler flow image (CDFI) in therapeutic effect of transcatheterarterial embolization (TAE) of hepatocellular carcinoma(HCC).MethodsCDFI findings on 36 HCCcases were evaluated and compared with angiography and CT before and after TAE.ResultsColorDoppler flow patterns in 34 cases(94.4%) before TAE fell into four(1) peritumoralpulsatile flow in the periphery of the tumor;(2) intratumoral pulsatile flow in thecentral area of the tumor;(3) peritumoral continuous flow in the periphery of thetumor;(4)intratumoral continuous flow in the central area of the tumor.Comparison withangiography,the feeding arteries within or near the tumor capsule were found in the firsttwo groups,negative in the other two.Two weeks after TAE,color Doppler flow was stillidentified in 21 (58.3%) of the 36 patients and angiography revealed recirculation oftumors' feeding artery in 28 cases who underwent angiography once more.The patterns ofCDFI and the numbers of feeding artery were associated to the tumor size.ConclusionCDFIis reliable in evaluating the therapeutic effect of TAE and follow up for seekingrecurrence.
