64排螺旋CT胸部三合一扫描在急性胸痛检查中的应用

目的探讨64排螺旋CT胸部“三合一”扫描在急性胸痛检查中的应用。方法利用飞利浦64排螺旋CT进行胸部“三合一”增强扫描，57例获得原始横断面图像后，采用容积再现（VR）、最大密度投影（MIP）、多平面重建（MPR）技术处理图像，29例与DSA结果进行比较。结果所有病例CT三合一扫描一次检查均获成功，显示冠状动脉狭窄25例，闭塞1例；急性肺动脉栓塞3例；主动脉夹层6例；房间隔缺损2例。结论64排螺旋CT胸部一次扫描获得冠状动脉、主动脉及肺动脉重建图像，对于诊断冠心病、主动脉夹层、肺动脉栓塞等导致急性胸痛的常见疾病是一种方便可靠的无创检查方法，具有重要临床应用价值。

Application of 64-slice spiral CT chest three-in-one scan in examination of acute chest pains

Objective To study the value of 64-slice spiral CT chest three-in-one scan in examination of acute chest pains. Methods The orginal enhanced axial CT images of 57 patients were obtained with Philips 64-slice spiral CT, and the images were then reconstructed on the extended brilliance workspace (EBW) with volume rendering (VR), maximum intensity projection (MIP) and multiplannar reconstruction (MPR), respectively. Among the 57 patients, 29 underwent DSA, and the results were compared with that of CTA.Results All three-in-one scan examinations obtained success. The acute myocardial infarction were showed in 25 cases, coronary artery occlusion was showed in 1 case. Pulmonary artery embolism was found in 3 cases, and aortic dissection in 6 cases, atrial septal defect in 2 cases. Conclusion 64-slice spiral CT chest one scan can timely and accurately sieve patients with chest pain suffered from whether coronary arteries or other acute severe lesions, and is an accurate and non-invasive technique promising important clinical practice value.
