摘    要:目的总结经皮内镜下胃空肠造瘘术SAP患者的护理体会。方法对14例重症急性胰腺炎患者行PEG和PEJ的护理情况进行回顾性分析。结果14例患者均造瘘成功,引流通畅,成功率100%,并顺利实施肠内营养。2例术后出现低热,造瘘口周围皮肤局部红肿,经换药及抗生素治疗2d体温恢复正常。1例使用胃造瘘注入流质饮食时造成导管堵塞,经用糜蛋白酶8000U+温生理盐水50ml反复冲洗后导管得以疏通。结论经皮内镜下空肠造瘘术虽是微创手术,但其围术期及术后肠内营养的护理十分重要。

Abstract:Objective Summarize nursing experience in patients with severe acute pancreatitis treated with percutaneous endoscopic gastrotomy( PEG )or percutaneou endoscopic jejunostomy(PEJ).Methods 14 cases of patients with percutaneous endoscopic gastrotomy (PEG)or percutaneou endoscopic jejunostomy (PEJ)were retrospective analyzed.Results The PEG or PEJ operation was success in 14 patients and draining was unobstructed, the rate of success was 100%, these patients were given enteral nutrition. The patients with slight complications, including 2 cases of low fever after operation because of infection of incisional wound with local skin red swelling,after 2 days of dressing change and anbiotic treatment, the two patients body temperature got recover to normal. Duct obstruction in 1 case for using liquid diet by stoma, after washing by chymotrypsin 8000 unit and warm normal saline 50ml, the duct was deoppilation.Conelusiou The PEG or PEJ is minimally invasive operation,but the nutrition nursing is important for perioperation and post-operation.
