HLA-DR抗原及HPV16/18E6蛋白在宫颈癌组织的表达及意义

摘    要:目的:通过检测HLA-DR抗原及HPV16/18E6蛋白在宫颈癌组织中的表达情况,探讨由HLA-DR抗原介导的免疫应答在感染高危型人乳头瘤病毒(HPV)至发展为宫颈癌过程中的作用机制。方法:采用免疫组织化学SP法检测正常宫颈、宫颈上皮内瘤病变(CIN)及宫颈癌组织中HLA-DR抗原及HPV16/18E6的表达情况。结果:HLA-DR抗原的阳性表达率在正常宫颈、CIN、宫颈癌组织中分别为37.5%、73.3%、81.8%,而HPV16/18E6蛋白的阳性表达率则分别为37.5%、60.0%、75.8%,两者在3组的表达差异显著,P均<0.05。HLA-DR抗原及HPV16/18E6蛋白在宫颈癌组织中的阳性表达率随临床分期、分化程度及淋巴结是否转移而不同,但无显著性差异,P均>0.05。结论:HLA-DR抗原递呈病毒抗原给T细胞引起的免疫应答可能在宫颈癌的发生发展过程中起重要的作用。

Abstract:Objective:To investigate the expressions of HLA-DR antigen and HPV-16/18E6 protein in cervical cancer,and to explore the role of immune response mediated by HLA-DR antigen during the process of high-risk HPV infection developing into cervical cancer. Methods:Immunohistochemical SP method was used to examine the expressions of HLA-DR antigen and HPV-16/18E6 protein in tissues sampled from normal cervix,cervical intraepithelial neoplasia (CIN),and cervical cancer. Results:The positive expression rates of HLA-DR antigen and HPV-16/18E6 protein were 37.5%,73.3%,and 81.8% and 37.5%,60.0%,and 75.8% in normal cervix,CIN,and cervical cancer,respectively. There were significant differences in positive expression rates of HLA-DR antigen and HPV-16/18E6 protein both between normal cervix and CIN and between normal cervix and cervical cancer (P < 0.05). The positive expression rates of HLA-DR antigen and HPV-16/18E6 protein were different between cervical cancers of different clinical stages,between cervical cancers of different differentiation degrees,between cervical cancer with and without lymph node metastasis,but the differences were not significant. Conclusion:The immune response mediated by HLA-DR antigen which carries viral antigen to T cells may play an important role in the occurrence and development of cervical cancer.
