骨水泥在骨折椎体的弥散情况对椎体成形术治疗效果的影响

摘    要:目的分析骨水泥在骨折椎体的弥散情况对椎体成形术治疗效果的影响。方法选取2014年11月到2017年12月我院收治的100例不典型性老年骨质疏松性胸腰椎压缩性骨折患者作为研究对象,按照影像学结果将患者分为实验组(n=50)与常规组(n=50)。实验组患者为骨水泥在骨折椎体内充分弥散的患者,常规组患者为骨水泥在骨折椎体内弥散不佳的患者。所有患者均采用相同的手术治疗方案,比较两组患者治疗前后的椎体前缘高度、Cobb角、机体功能评分。结果治疗前,两组患者的Cobb角、椎体前缘高度比较,差异无统计学意义(P>0.05);治疗后,实验组患者的Cobb角、椎体前缘高度以及机体功能评分均显著优于常规组,差异具有统计学有意义(P<0.05)。结论骨水泥在骨折椎体的弥散情况对椎体成形术的治疗效果影响明显,骨水泥弥散不佳可导致椎体康复效果不理想,存在发生后凸畸形的可能,临床中应当高度重视。

Abstract:Objective To analyze the impact of bone cement diffusion in fractured vertebral body on the curative effect of vertebroplasty.Methods 100 elderly patients with atypical osteoporotic thoracolumbar compression fracture admitted to our hospital from November 2014 to December 2017 were selected as study objects and were divided into experimental group(n=50) and conventional group(n=50) according to the imaging results. Patients with poor bone cement diffusion in fractured vertebral body were set as the conventional group, and patients with good bone cement diffusion in fractured vertebral body were set as the experimental group. All patients were treated with the same surgical regimen, and the anterior vertebral height, Cobb angle and body function score were compared between two groups.Results Before treatment, no statistical difference was found between two groups in the anterior vertebral height and Cobb angle(P>0.05);After treatment, the anterior vertebral height, Cobb angle and body function score of experimental group were significantly better than those of conventional group(P<0.05). Conclusions Bone cement diffusion in fractured vertebral body has significant impact on the curative effect of vertebroplasty. Poor bone cement diffusion can lead to unsatisfactory vertebral rehabilitation and the possibility of kyphosis, which should be taken seriously in clinic.
