急诊胃镜在不同年龄段上消化道出血患者中的诊治意义

目的分析急诊胃镜在不同年龄段上消化道出血患者中的诊治意义。方法选择2017年9月至2019年8月我院收治的100例上消化道出血患者,均行胃镜检查。记录胃镜诊断病因结果,分析不同年龄段患者的病因分布差异,并对比急诊胃镜治疗与非急诊胃镜治疗患者的恢复情况。结果经胃镜分析病因显示消化性溃疡52例,急性胃黏膜病变19例,肝硬化食管胃底静脉曲张破裂出血14例,消化道肿瘤9例,其他6例,确诊率为100.00%。中青年组与老年组的消化性溃疡、消化道肿瘤病因分布比较有统计学差异(P<0.05)。100例患者经胃镜下治疗,均治疗成功。急诊胃镜患者的止血时间、住院时间短于非急诊胃镜患者(P<0.05)。结论中青年与老年上消化道出血病因分布有一定差异,可通过胃镜明确病因并进行急诊胃镜下治疗,快速止血。

Significance of Emergency Gastroscopy in the Diagnosis and Treatment of Upper Gastrointestinal Bleeding in Different Age Patients

Objective To analyze the significance of emergency gastroscopy in the diagnosis and treatment of upper gastrointestinal bleeding in different age patients.Methods 100 cases of patients with upper gastrointestinal bleeding admitted to our hospital from September 2017 to August 2019 were given gastroscopy.The etiological results of gastroscopy diagnosis were recorded,and the difference of etiological distribution among different age groups was analyzed,and the recovery of patients receiving emergency gastroscopy and non-emergency gastroscopy was compared.Results Analysis of etiology by gastroscopy showed 52 cases of peptic ulcer,19 cases of acute gastric mucosal lesions,14 cases of cirrhosis esophageal and gastric variceal bleeding,9 cases of gastrointestinal tract tumors and 6 other cases,with the diagnostic rate of 100.00%.Statistical difference was found in the etiological distribution of peptic ulcer and gastrointestinal cancer between the young and middle-aged group and the elderly group(P<0.05).100 patients were treated successfully by gastroscopy.The hemostasis time and hospitalization time of emergency gastroscopy patients was shorter than that of non-emergency gastroscopy patients(P<0.05).Conclusions The etiological distribution of upper gastrointestinal bleeding in young and middle-aged patients,and elderly patients is different.The etiology of upper gastrointestinal bleeding can be determined by gastroscopy and treated by emergency gastroscopy to stop bleeding quickly.
