凶险型与普通型前置胎盘对孕产妇的不良影响分析

目的分析凶险型与普通型前置胎盘对孕产妇的不良影响。方法选取2014年1月至2018年12月我院收治的60例凶险型前置胎盘产妇作为研究组,选取同期我院收治的60例普通型前置胎盘产妇作为对照组,对比两组产妇的手术情况及新生儿结局。结果研究组的产后出血、胎盘植入、凝血功能异常、输血、转入ICU及子宫切除比例分别为61.67%、40.00%、50.00%、58.33%、16.67%、15.00%,均明显高于对照组的18.33%、6.67%、16.67%、13.33%、1.67%、1.67%,差异有统计学意义(P<0.05)。两组的新生儿结局比较无统计学差异(P>0.05)。结论与普通型前置胎盘相比,凶险型前置胎盘对孕产妇的危害更大,临床应严格把握剖宫产手术的适应证,提倡阴道试产,降低剖宫产率及相关并发症风险。

Analysis on Adverse Effect of Dangerous and Common Placenta Previa on Puerperas

Objective To analyze the adverse effect of dangerous and common placenta previa on puerperas.Methods 60 cases of puerperas with dangerous placenta previa admitted to our hospital from January 2014 to December 2018 were selected as the study group,and 60 cases of puerperas with common placenta previa in our hospital at the same time were selected as the control group.The operation conditions and neonatal outcomes were compared between the two groups.Results The rates of postpartum hemorrhage,placenta implantation,coagulation dysfunction,blood transfusion,ICU transfer and hysterectomy were respectively 61.67%,40.00%,50.00%,5&33%,16.67%and 15.00%of the study group were significantly higher than 18.33%,6.67%,16.67%,13.33%,1.67%and 1.67%of the control group,with statistical differences(P<0.05).No statistical difference was found in the neonatal outcomes between the two groups(P>0.05).Conclusions Compared with common placenta previa,dangerous placenta previa is more harmful to puerperas.Clinical indications of cesarean section should be strictly grasped,and the vaginal trial delivery should be advocated,so as to reduce the cesarean section rate and risk of related compilations.
