重型颅脑损伤行鼻肠管早期肠内营养辅以肠外营养的应用

目的观察鼻肠管早期肠内营养辅以肠外营养对重型颅脑损伤患者的疗效。方法将84例重型颅脑损伤患者随机分为观察组与对照组各42例,观察组留置鼻肠管,对照组留置鼻胃管,均给予早期肠内营养辅以肠外营养的营养支持方式,观察两组生活指标、胃肠道耐受及并发症情况。结果营养支持后7d血清总蛋白、清蛋白两组间差异无统计学意义（P〉0.05）;两组患者营养支持后7d血糖水平间差异有统计学意义（P〈0.01）。1周内并发症发生情况：观察组42例中发生消化道出血5例,反流2例,腹泻4例,腹胀2例。对照组42例中发生消化道出血9例,反流19例,腹泻9例,腹胀17例。结论重型颅脑损伤病患者鼻肠管早期肠内营养辅以肠外营养支持方式,可以明显改善营养状况,减少并发症的发生。

Nasal Tube of Early Enteral Nutrition Supplemented with Parenteral Nutrition Support in Severe Head Injury in Clinical Research

Objective To observe the nasal tube of early enteral nutrition supplemented parenteral nutrition on patients with severe brain injury treatment.Methods 84 cases of patients with severe brain injury were randomly divided into experimental group and control group 42 cases,the experimental group indwelling nasal tube,the control group indwelling nasogastric tube,were given early enteral nutrition supplemented with the nutritional support over-the-counter nutritional approach observed two groups of life indicators,gastrointestinal tolerance and complications of the situation.Results The nutritional support 7d serum protein after an upward trend,but no difference in the two groups (P＞ 0.05);two groups of blood glucose than those of nutritional support before the decline in difference between the two groups was significant (P＜0.01);the experimental group concurrent Disease incidence was significantly lower than control (P＜0.05).Conclusion The severe head injury patients with nasal tube of early enteral nutrition supplemented with over-the-counter nutritional support,it can be significantly improved nutritional status,reduce complications.
