阻塞性睡眠呼吸暂停低通气综合征儿童认知行为异常与血清BDNF的相关性研究

摘    要:目的 通过研究儿童阻塞性睡眠呼吸暂停低通气综合征(OSAHS)认知行为异常与血清BDNF的关系,阐释儿童OSAHS认知行为异常可能的发病机制。方法 选择经整夜多导睡眠监测诊断的40例OSAHS及22例单纯鼾症的4~12岁儿童为研究对象,并以22例健康儿童为对照组。采用Conners简明症状问卷评定儿童行为问题,酶联免疫吸附方法检测各组儿童血清中脑源性神经营养因子(BDNF)的浓度。结果 OSAHS及单纯鼾症组存在行为问题的比率高于健康对照组(P<0.01), 而两组之间无统计学差异(P>0.05)。OSAHS组、单纯鼾症组BDNF水平低于健康对照组(P<0.01), 而两组之间无统计学差异(P>0.1)。OSAHS组伴有行为异常者BDNF水平低于不伴有行为异常者(P<0.01) ;单纯鼾症组伴有行为异常者BDNF水平低于不伴有行为异常者(P<0.05)。结论 OSAHS及单纯鼾症儿童较健康儿童有较高的认知行为异常发生率。血清BDNF水平的降低与OSAHS及单纯鼾症儿童的认知行为问题具有相关性。

Abstract:Objective To explore the possible mechanism of cognitive function deterioration in children with obstructive sleep apnea-hypopnea syndrome(OSAHS). Methods The behavior problems and brain-derived neurotrophic factor (BDNF) were measured in 40 children with OSAHS and 22 children with habitual snoring who underwent overnight polysomnography. 22 healthy children served as the control group. Results The ratio of abnormal behavior in OSAHS and habitual snoring children was higher than that in healthy group(P<0.01), while there was no difference between the two groups (P>0.05). The content of BDNF in OSAHS and habitual snoring children was lower than the healthy group(P<0.01), while there was no difference between the two groups (P>0.1). The content of BDNF in OSAHS children accompanied by abnormal behavior was lower than that without abnormal behavior(P<0.01), and the difference was also found in habitual snoring children(P<0.05). Conclusion OSAHS and habitual snoring children have more behavior problems, BDNF levels are lower in children with OSAHS and habitual snoring, and the levels of BDNF are related to the abnormal behavior in these children.
