肠易激综合征与胃肠激素的关系

目的　探讨胃肠激素在肠易激综合征 (IBS)患者血浆及乙状结肠粘膜中含量的变化及临床意义。方法　采用放射免疫法测定正常对照组及IBS患者血浆和乙状结肠粘膜胃动素 (MTL)、血管活性肠肽(VIP)、生长抑素 (SS)、胆囊收缩素 (CCK)的含量。结果　IBS腹泻型患者血浆及乙状结肠粘膜中MTL含量显著高于正常组及IBS便秘型组 (P <0 .0 1) ;便秘型组IBS患者乙状结肠粘膜VIP的含量高于正常组及腹泻型组IBS(P <0 .0 5 ) ,而各组间血浆中VIP无明显差异 (P >0 .0 5 ) ;血浆及乙状结肠粘膜SS的含量在便秘型组IBS中显著高于正常组及腹泻型组 (P <0 .0 1～ 0 .0 5 ) ;血浆CCK在腹泻组也明显高于正常组及便秘型组 (P <0 .0 1) ,在乙状结肠粘膜中各组间无明显不同 (P >0 .0 5 )。结论　四种胃肠激素可能在IBS的发病过程中起一定作用。

Relationship between gastrointestinal hormones and irritable bowel syndrome

Objective To investigate the change and clinical significance of gastrointestinal hormones in the plasma and sigmoid mucosa of the patients with irritable bowel syndrome(IBS).Methods The motilin(MTL), vasoactive intestinal peptide(VIP),somatostatin(SS) and cholecystokinin(CCK) in plasma and sigmoid mucosa of the normal group and the IBS group were measured by the radioimmunoassay.Results The plasma and sigmoid mucosa MTL was significantly higher in IBS patients with diarrhea than that in contrpls and IBS patients with constipation (P<0.01) .The sigmoid mucosa VIP was significantly higher in the IBS group with constipation than in contrpls and IBS group with diarrhea(P<0.05),while the plasma VIP was not found to be significantly different (P>0.05) .The plasma and sigmoid mucosa SS was significantly higher in IBS group with constipation than that in contrpls and IBS group with diarrhea (P<0.01-0.05).The plasma CCK was significantly higher in the IBS group with diarrhea than that in contrpls and IBS group with constipation(P<0.01),while the sigmoid mucosa CCK was not significantly different(P>0.05).Conclusion The changes of these four gastrointestinal hormones may play some roles in the pathogenesis of IBS.
