甲状腺钙化灶对恶性结节诊断的临床意义

摘    要:目的 甲状腺结节是临床常见疾病.文中旨在研究甲状腺结节钙化对甲状腺恶性肿瘤诊断的临床意义,提高恶性甲状腺结节的诊断率.方法 对比247例甲状腺结节术前超声发现及术后病理诊断.结果 247例甲状腺结节患者中,良性病变200例(80.97%),恶性病变47例(19.03%).良性结节钙化率23.0%(46/200),恶性结节钙化率59.6%(28/47),2组间差异有显著统计学意义(χ2=24.261,P<0.001);良性结节中微钙化发生率为9.5%,恶性结节微钙化发生率为46.8%,差异亦有显著统计学意义(χ2=38.101,P<0.001).结论 甲状腺钙化尤其是微钙化对诊断甲状腺恶性肿瘤有重要意义.

Abstract:Objective Thyroid nodules are common clinical disorders.The present study was to evaluate the significance of sonographically detected calcification of thyroid nodules in the diagnosis of thyroid malignancy so as to improve the diagnosis of malignant thyroid nodules.Methods We retrospectively analyzed and compared the results of pre-operative ultrasonography and post-operative pathology of 247 patients with thyroid nodules.Results Of the 247 cases of thyroid nodules,200(80.97%) were benign and 47(19.03%) malignant.The rate of thyroid calcification was significantly lower in the benign than that in the malignant samples(23.0% vs 59.6%,χ2=24.261,P<0.001),and so was the rate of micro-calcification(9.5%vs 46.8%,χ2=38.101,P<0.001).Conclusion Thyroid calcification,particularly micro-calcification,is an important characteristic of thyroid malignancy.
