食管鳞状细胞癌中MMP-2和p63的表达及其临床意义

目的目前对食管癌的早期诊断尚无特异性敏感性较高的标志物。基质金属蛋白酶(matrix metalloproteinase,MMP)-2在肿瘤转移过程中起关键作用。p63可能在鳞状细胞癌发生中起癌基因的作用。文中观察MMP-2和p63蛋白在食管鳞状细胞癌中的表达及临床意义,探讨其作为食管癌早期诊断标志物的应用价值。方法收集手术切除的134例食管鳞状细胞癌标本,每例标本均取食管正常黏膜组织、癌旁组织和癌灶组织,用免疫组化EnVision法检测MMP-2和p63的表达。结果 MMP-2和p63在正常食管黏膜、癌旁和癌灶3种组织中均呈递增性表达,MMP-2在3种组织中表达水平两两比较,P值分别为0.010、<0.001、0.005。p63在3种组织中表达水平两两比较P值分别为0.019、<0.001、0.016。MMP-2和p63蛋白表达与肿瘤浸润深度、淋巴结转移及临床分期均密切相关。食管鳞状细胞癌组织中MMP-2和p63的表达呈正相关。结论 MMP-2和p63蛋白表达水平均与食管鳞癌恶性程度密切相关,2种蛋白可能具有协同作用,联合检测对食管癌的诊断更有意义。

Expressions of matrix metalloproteinase-2 and p63 in esophageal squamous cell carcinoma and their clinical significance

Objective Currently there are not yet any higher specificity and sensitivity markers for the diagnosis of esophageal carcinoma.Matrix metalloproteinase 2(MMP-2) plays a key role in cancer metastasis,and p63 may assume the role of a cancer gene in the development of squamous cell carcinoma.This study looks into the expressions of MMP-2 and p63 in 134 cases of esophageal squamous cell carcinoma,their clinical significance,and their application value as biological markers in the diagnosis of esophageal carcinoma.Methods We collected specimens of esophageal squamous cell carcinoma from surgical resection,and detected the expressions of MMP-2 and p63 proteins in the peficancerous,cancerous and normal mucosa tissues from each specimen by EnVision immunohistochemistry.Results Both MMP-2 and p63 were expressed in the normal esophageal mucosa,peficancerous and cancerous tissue in an increasing manner.By pairwise comparison,the P values of the MMP-2 expression were 0.010,<0.001 and 0.005,and those of the p63 expression were 0.019,<0.001 and 0.016 in the above three types of tissues,respectively.The expressions of MMP-2 and p63 were closely associated with the infiltration degree,lymph node metastasis and clinical stages of cancer,and positively correlated with each other in esophageal squamous cell carcinoma tissues.Conclusion The expression levels of MMP-2 and p63 are closely related with the malignancy degree of esophageal squamous cell carcinoma.The two proteins may have a synergetic effect,and so the detection of the expressions of them both is of more significance for the diagnosis of esophageal carcinoma.
