不同年龄组自发性蛛网膜下腔出血的临床特点分析

目的:探讨自发性蛛网膜下腔出血在不同年龄患者中的临床特点。方法:76例原发性蛛网膜下腔出血(SAH)按照年龄段分组并将患者临床表现、并发症、误诊、漏诊以及预后等情况进行对照分析。结果:老年组患者头痛、呕吐、脑膜刺激征、意识障碍等典型的SAH症状均不如青壮组明显(P<0.05或0.01);总误诊率30.6%,明显高于青壮组(P<0.05);治疗总有效率77.8%,明显低于青壮组(P<0.05)。结论:老年SAH临床症状表现缺乏特异性,容易误诊、漏诊,预后不如青壮年,临床诊治老年SAH要给予高度的重视,防止误诊、漏诊,降低死亡率。

Clinical Features Analysis of Different Age Group Patients with Spontaneous Subarachnoid Hemorrhage

Objective:To investigate the clinical features of spontaneous subarachnoid hemorrhage in patients of different ages.Methods:Clinical manifestations,complications,misdiagnosis misdiagnosis and prognosis of 76 cases of primary subarachnoid hemorrhage(SAH) were compared in accordance with the grouping of the ages.Results:Typical symptoms of SAH such as headache,vomiting,meningeal irritation,disturbance of consciousness in elder patients were not as obvious as the young group(P<0.05 or 0.01);total misdiagnosis rate(30.6%) significantly higher than young group(P<0.05);total effective rate was 77.8%,lower than the young group(P<0.05).Conclusion:The clinical symptoms of SAH in the elder patients lacked specificity and easily misdiagnosed,the prognosis was not as good as young.The clinical diagnosis and treatment of elderly SAH should be given a high priority.
