PPD试验对肺结核疗效预测的研究

目的:评估PPD试验对肺结核治疗疗效的预测中的应用价值,为预测肺结核的疗效提供一种预测方法,并指导结核病的治疗。方法:将所有列入观察组的肺结核患者进行PPD试验,并根据皮试结果表现分为阴性反应组、阳性反应组以及强阳性反应组,分别给以抗结核治疗,观察不同PPD反应患者的痰菌转阴速度、空洞闭和速度以及疗程结束时病灶的吸收速度。结果:168例痰菌阳性初治的结核病患者,根据PPD试验的结果分为阴性组(Ⅰ组),阳性组(Ⅱ组),强阳性组(Ⅲ组)。经过6个月抗痨治疗后,Ⅰ组痰菌转阴率为76.92%,平均转阴时间为12.46周,病人疗程结束时,肺内病灶X线吸收占61.54%;Ⅱ组痰菌转阴率为94.79%,平均转阴时间为5.0周,病人疗程结束时,肺内病灶X线吸收占86.46%;Ⅲ组痰菌转阴率为100%,平均转阴时间为3.0周,病人疗程结束时,肺内病灶X线吸收占100%。上述各组之间比较有明显统计学差异,P<0.005。结论:PPD试验强阳性患者的治疗疗效优于PPD试验阳性的患者,PPD阳性的患者的治疗疗效优于PPD试验阴性的患者。可以通过PPD试验的结果来预测肺结核的治疗疗效。

Study of PPD Test in Prevision the Curative Effect of Pulmonary Tuberculosis

Objective:To evaluate the value applied of PPD test in prevision the curative effect of PTB,to find out a prevision method for detecting the effect of PTB,and to guide treatment. Methods:All patients with PTB were received PPD test,according to the outcome of test,The cases were divided into negative reaction group,positive reaction group and strong positive reaction group.All patients were received antituberculotic treatment.The rate of sputum corersion,cavity closing up and focus absorption were observed. Results:According to the outcome of test,168 cases with new bacteriological positive PTB were divided into negative reaction group(group 1),positive reaction group (group 2)and strong positive reaction group(group 3).After antituberculotic treatment for 6 months.The rates of sputum coversion of group 1,group 2 and group 3 were 76.92% ,94.79% and 100% respectively.The mean sputum coversion time of group 1,group 2 and group 3 were 12.46 weeks 5.0 weeks and 3.0 weeks respectively and the rates of focus absorption of group 1 ,group 2 and group 3 were 61.54%. 86.46% and 100% respectively.It showed significant differences statistically in three groups(p<0.005).Conclusion:The curative effect in PPD strong-positive patients was better than PPD positive patients,and the curative effect in PPD positive patients was better than PPD negative patients.According to the outcome of PPD test,we can previct the curative effect of PTB.
