无创双水平正压通气治疗急性左心衰竭

摘    要:目的 观察无创双水平正压通气(BiPAP)在短期内治疗急性左心功能衰竭(下称急性左心衰)的临床疗效.方法 将急性左心衰患者40例随机分为两组,对照组予常规抗心力衰竭药物治疗,BiPAP组在给予常规药物同时加用BiPAP呼吸机辅助治疗,分别观察记录两组患者的临床表现及治疗前后的血气、心率、呼吸频率等参数的变化,并作统计学分析.结果 BiPAP组总有效率为95%,对照组为65%,两组比较差异有统计学意义(P<0.05).与对照组比较,通气治疗后BiPAP组血气分析氧分压、二氧化碳分压、血氧饱和度,各指标显著改善(P<0.05).结论 BiPAP通气治疗急性左心衰能迅速改善患者的症状及低氧血症,是快速、有效的方法.

Abstract:Objective To assess the efficacy of noninvasive Bi-level positive airway pressure(BiPAP)ventilation on acute left heart failure.Methods Forty patients with acute left heart failure were divided into control group(conventional anti-heart failure drug treatment)and BiPAP group(conventional treatment ventilator assisted treatment).Arterial blood gases,respiratory rate(RR),heart rate and blood pressure were measured before and after treatment,and then statistical analysis was performed.Results The total efficiency rate of BiPAP group was 95%,while the total efficiency rate of control group was only 65%.There was statistical difference between two groups.As compared with that of control group,the arterial blood gas indexes(PaO2,PaCO2 and SaO2)in BiPAP group improved markedly(P<0.05).Conclusion Noninvasive BiPAP ventilation can improve symptoms and hypoxemia in patients with acute left heart failure.It is a feasible and effective method in the treatment of acute left heart failure.
