泌尿系感染常见病原菌分布及大肠埃希菌耐药性分析

摘    要:目的 了解本院泌尿系感染病原菌的分布及大肠埃希菌的耐药性,为临床合理用药提供依据.方法 采用API鉴定系统进行病原菌鉴定,用琼脂纸片扩散法(K-B法)进行药敏试验.结果 分离出病原菌282株,其中革兰阴性杆菌199株,占70.57%;革兰阳性球菌67株,占23.76%.检出率最高的是大肠埃希菌,其次为肠球菌和葡萄球菌等.大肠埃希菌对亚胺培南、头孢哌酮/舒巴坦、哌拉西林/他唑巴坦的耐药率较低,分别为1.32%、7.89%、11.18%;对复方新诺明、环丙沙星、哌拉西林的耐药率较高,分别为92.76%、80.26%、79.61%.结论 泌尿系感染主要致病菌为大肠埃希菌,临床医生应依据药敏试验结果合理使用抗生素.

Abstract:Objective To understand the distribution of main pathogenic bacteria and the status of drug resistance of Escherichia coli in urinary system infection in our hospital and to provide the basis for reasonable drug use.Methods The bacteria identification system of France Biomerieux API was used in the bacterial identification.Drug sensitive test was performed with paper stip method.Results 282 strains of pathogenic bacteria were separated from the urine samples of 1 068 cases and the number of strains of Gram negative bacilli was 199,the Gram positive cocci was 67.The positive rate of Escherichia coli was the highest followed by enterococcus and staphylococci.The drug resistant rate of Escherichia coli was the lowest to imipenem followed by cefoperazone/sulbactam and piperacillin/tazobactam.The drug resistant rate of Escherichia coli was the highest to sulfamethoxazole/trimethoprim followed by ciprofloxacin and piperacillin.Conclusion The urinary infections are mainly caused by Escherichia coli.Clinicians must use antibiotics reasonably depending on the results of drug sensitive tests.
