血管内支架成形术治疗颈动脉狭窄患者的疗效观察

摘    要:目的探讨应用血管内支巢成形术治疗症状性颈动脉狭窄患者的可行性及安全性。方法对72例症状性颈动脉狭窄患者予以颈动脉支架成形术治疗。结果全组技术成功率100%,术前平均狭窄率为81.7%,术后残余狭窄率〈20%。术后1周内患者症状及神经功能缺损体征有所改善。术中及术后发生心率减慢23例,血压下降15例,血管痉挛7例,发生脑栓塞、高灌注综合征各1倒。随访3个月-1年,共有9例复查了DSA,1例出现轻度再狭窄;未发现支架移位,未发生缺血性脑卒中。结论血管内支架成形术是治疗症状性颈动脉狭窄安全、有效的方法,严格掌握适应证和熟练操作可降低手术风险。

Curative observation of stent angloplasty for treating the patients with carotid artery stenosis

Abstract:Objective To observe the feasibility and safety of stent angloplasty for treating the patients with symptomatic carotid artery stenosis. Methods 72 patients who were diagnosed with symptomatic carotid artery stenosis were performed carotid artery stent angloplasty. Results The successful rate of stent implantation was 100%. 42 patients underwent stent angloplasty with a mean pre-procedural carotid stenosis of 81.7% and the residual stenosis was less than 20%. The clinical symptoms of all patients were disappear, the neurological function deficit was recover in all patients during a week. There were 23 cases with heart rate slow down during or after operation, the blood pressure dechned in 15 cases, and there were 7 patients with vasospasm ,as well as one case with cerebral embolization, one case with sydrome of high perfusion. During flowing up for 3 months to 1 years, 9 patients were diagnosed again by DSA, only one case was found mile stenosis, and no one case was found the shift of stent or cerebral infarction. Conclusion Stent angloplasty is a safety and efficient method for treatment synaptomatio carotid artery stenosis. Lowering the risk of operation is done by handling indication seriously and operation skillfully.
