低浓度罗哌卡因腰麻在经尿道膀胱肿瘤电切老年患者中的应用

目的 观察低浓度罗哌卡因腰麻用于经尿道膀胱肿瘤电切老年患者的可行性和安全性。方法 选择行经尿道膀胱肿瘤电切的老年患者60例,年龄65~89岁,随机分为低浓度腰麻组（A组）、常规腰麻组（B组）,每组30例,分别于蛛网膜下腔注入0.25%罗哌卡因或0.5%罗哌卡因1.2ml。观察指标为感觉和运动阻滞的时间,麻醉前及注药平躺后即刻、麻醉后15min及麻醉后30min的平均动脉压、心率和SpO2值。结果 两组麻醉效果均达优良,麻醉后B组患者血压、心率较A组显著降低,两组麻醉起效时间无差异,麻醉维持时间A组明显减少。结论 低浓度罗哌卡因腰麻用于经尿道膀胱肿瘤电切老年患者时麻醉效果确切,并发症较少。

The application of low concentration ropivacaine spinal anesthesia in patients undergoing transurethral resection of bladder tumor

Objective To observe the clinical effective and safety of spinal anesthesia with low concentration ropivacaine in aged patients undergoing transurethral resection of bladder tumor （TURBT）. Methods 60 cases of aged patients （age range of 65 to 89 years old） who would accept TURBT were randomly into two groups, 30 cases in each group. Patients in group A received subarachnoid injection of 0. 25 % ropivarcine, and patients in group B subarachnoid injection of 0.5 % ropivarcine. Sensory never and motor never block time was observed. The general body parameters （MAP, HR, SpO2） were also recorded during process of spinal anesthesia. Results Compared with group A, MAP and HR of patients in group B decreased significantly after anesthesia. The maintain time of never block of patients in group B significantly longer than in group A. Conclusions Low concentration ropivarcine spinal anesthesia is effective and safety in aged patients strategy TURBT.
