乌拉地尔治疗慢性肺心病难治性心力衰竭36例临床观察

目的　观察乌拉地尔治疗慢性肺心病难治性心力衰竭的临床效果。方法　将 68例慢性肺心病难治性心衰患者随机分为治疗组 (3 6例 )和对照组 (3 2例 )。对照组采用抗菌、祛痰、平喘、利尿、强心 ,氧疗等常规治疗法 ,治疗组在常规治疗法的基础上停用利尿、强心剂 ,予乌拉地尔 10 0～ 12 5mg加入 5 %葡萄糖注射液 2 5 0ml中静脉滴注 ,滴速 10 0～2 0 0 μg/min ,每日 1次 ,疗程 7d。注意监测血压 ,使血压维持在 12 .0～ 8.0kPa以上 ,并根据血压调整滴速。 结果　治疗组总有效率为 94.44 % ,对照组为 5 3 .13 % ,两组比较差异有高度显著性 (χ2 =15 .42 6,P <0 .0 1)。无不良反应。结论　采用常规治疗法并用乌拉地尔治疗慢性肺心病难治性心力衰竭疗效显著。

Efficacy of urapidil in treatment of 36 patients with refractory heart failure due to chronic pulmonary heart disease

Objective To evaluate the clinical efficacy of urapidil in treatment of refractory heart failure due to chronic pulmonary heart disease.Methods Patients with refractory heart failure due to chronic pulmonary heart disease were allocated to a treated group (n=36) and a control group (n=32). In the control group, patients received the traditional treatment with antibiotics, expectorants, anti-asthmatic agents, diuretics, cardiotonic agents and oxygen therapy. In the treated group, on the basis of the traditional treatment, patients were given urapidil 100～125mg plus in 5% glucose solution by intravenous drip, at a drip velocity of 100～200 μg, once every day for seven days. Pay attention to monitoring the blood pressure, keeping the blood pressure over 12.0～8.0kPa and regulating the drip velosity according to the blood pressure. Results The total effective rate in the treated group was 94.44%, and the control group was 53.13%. In comparison of the total effective rate between these two groups, there was statistically significant difference, χ2=15.426, P<0.01. No side effect. Conclusion The traditional therapy combined with urapidil is markedly effective in treatment for refractory heart failure due to pulmonary heart disease.
