孤立性纤维性肿瘤临床病理及免疫组化特点

目的探讨孤立性纤维性肿瘤（sofitary fibrous tumor,SFT）的临床、病理组织学、免疫组化特征,提高对该病的认识。方法对3例SFT病例进行大体观察、病理组织学及免疫组化观察。结果 3例SFT分别发生于右腰部、左下肺、左颞部,临床表现为肿块明显增大或有压迫所致的症状,肿块大小3cm×2.5cm×4cm～15cm×13cm×12cm,光镜下肿瘤细胞由梭形细胞构成束状、编织状排列,间质内有多少不一的胶原纤维,无特殊的组织构型及形态的多样性为其组织学特点。免疫表型：CD34、Vimentin、CD99、bcl-2均为阳性,S-100、desmin、SMA、EMA均阴性。结论 SFT是一组形态多样、无特殊组织构型及有多少不一胶原纤维的梭形细胞肿瘤,其特殊的免疫表型,有助于对该病的诊断,但SFT的确诊必须结合组织形态和免疫组化共同完成。

Clinicopathological and Immunohistochemical Characteristics of Solitary Fibrous Tumor

Objectives To investigate the characteristics of solitary fibrous tumor（SFT）by clinicopathological,histological and immunohistochemical analysis,and to increase the knowledge about SFT.Methods Three cases of SFT were observed by the general pathology,histopathology and immunohistochemistry.Results 3 cases of SFT occurred in right waist,left lower lung and left temporal lobe,respectively,with the clinical semiology of obviously increasing tumors and oppressive symptoms.The tumors ranged 3cm×2.5cm ×4cm to 15cm×13cm×12cm.Under microscope,tumors were composed of spindle cells that were arranged in bunchiness and weaving,and different quantity of collagen in stroma.The histopathological characteristics of SFT were unspecific organizational configurations and morphological multiformity.In immunophenotype,SFT was positive for CD34,Vimentin,CD99 and bcl-2,but negative for S-100,desmin,SMA and EMA.Conclusion SFT is a type of spindle cell tumor that exhibits morphological multiformity,unspecific organizational configurations and contains different quantity of collagen.Its specific immunophenotype would help us to diagnosis.The conclusive diagnosis must combine histopathological and immunohistochemical characteristics.
