颈部假性动脉瘤的诊断与处理

目的讨论颈部假性动脉瘤的诊断与处理。方法对9例颈部假性动脉瘤患者的病历资料进行回顾性分析。结果4例儿童患者为炎症诱发,2例颅内段患者为外伤所致,3例老年癌症患者颈部假性动脉瘤的产生与颈部放疗或肿瘤侵蚀有关。8例出现咽腔、鼻腔或颈部大出血,7例进行了动脉结扎或DSA栓塞术。9例患者中4例死亡,其中2例因瘤体溃破大出血而来不及抢救死亡;5例痊愈,均未出现明显并发症。结论颈部假性动脉瘤主要由炎症、外伤、放射线或者肿瘤侵蚀引起,假瘤破裂大出血是病死率高的主要原因,及早诊断和有效的结扎或栓塞病灶动脉是挽救患者生命的关键。

Diagnosis and Treatment of Cervical Pseudoaneurysm

Objective To explore the diagnosis and treatment of cervical pseudoaneurysm.Methods The data of 9 patients with cervical pseudoaneurysm was analyzed retrospectively.Results Inflammation was the cause of 4 children cases,2 cases were caused by injury,3 cancer patients were related with tumour and radiotherapy.Massive bleeding in pharynx,nasal cavity of neck skin were occurred on 8 cases,4 cases among them were died.Artery ligation and DSA embolism were performed in 7 cases,5 cases were cure without any complications.Conclusions These suggest that cervical pseudoaneurysm was mainly caused by inflammation,injury and radiotherapy or tumour erosion.Burst and bleeding of cervical pseudoaneurysm is the cause of high mortality.To improve the knowledge and vigilanve for this disease,early diagnosis and effective ligation and embolism of the artery are key methods to rescue the patients.
