速避凝对照小剂量普通肝素治疗急性损伤性DIC的临床观察

摘    要:目的评估低相对分子质量肝素(LMWH)速避凝在急性损伤性弥散性血管内凝血(DIC)的临床疗效。方法应用速避凝对照小剂量普通肝素治疗40例急性损伤性DIC。20例采用速避凝治疗,0.1-0.4ml皮下注射,每日1-2次(75-150u/kg.d-1),对照组20例采用常规小剂量普通肝素治疗,剂量为12.5-50mg,每日分1-3次(100-250u/kg.d-1)注射。结果速避凝组有效率(75%)比小剂量普通肝素组(65%)高,但无统计学意义。速避凝组出血率(30%,6例)低于小剂量普通肝素组(65%,13例),速避凝组死亡率明显低于小剂量普通肝素组(45%,vs65%,9例vs13例)。结论LMWH速避凝治疗急性损伤性DIC安全有效。

Abstract:Objective A randomized study was carried out involving in acute injured disseminated in-travascular coagulation to assess the efficacy and safety of LMWH (fraxiparine). Methods 20 cases were challenged by fraxiparine subcutaneously injection principally for average 2. 65 days at doses of 0. 1 - 0. 4 ml (2 500 - 10 000AXaIU/d,75 - 150 u/kg. d-1) , and controlled by 20 cases with low dose regular heparin 100 -250 u/(kg. d). Results 9 patients (45%) died of their underlying diseases or complications other than DIC in LMWH group, but control group had 13 patients death (65%). Hemorrhagic side effects occurred in 6(30%) and 13(65%) cases of groups fraxiparin and regular heparin respectively. Control group have more fatal bleeding (5 versus 1) and 2 fibrinolytic patients. Remarkable improvement was observed in 75% in the LMWH group and 65% in the regular heparin group, but statistics show no significant difference. Conclusion Its demonstrate LMWH (fraxiparine) is effective and saver in acute injured DIC.
