尿红细胞直径测量对血尿诊断的临床价值

目的　探讨一种简便、可靠的鉴别血尿来源的实验诊断方法。方法　对 5 7例血尿 (肾小球性血尿 36例 ,非肾小球性血尿 2 1例 )患儿的尿红细胞分别进行直径测定、相差显微镜检查及容积曲线测定 ,并对其鉴别血尿来源的敏感性及特异性进行了分析比较。结果　三种方法中 ,直径法无论在敏感性及特异性均优于其它两种方法。结论　尿红细胞直径测定用于鉴别血尿来源操作简便、结果可靠 ,适宜基层医院推广。

Clinical Values of Measuring the Diameter of Urinary Red Blood Cells in Diagnosis of Hematuria

Objective To explore a kind of handy and reliable experimental diagnostic method for differentiating hematuria.Method Red blood cells in urine from 57 children with hematurin(36 cases were glomerular hematuria and 21 cases were non-glomerular hematuria) were respectively examined with three kinds of method:diameter measurement,shape observation under phase-contrast microscope and the capacity-curve determination.The specificity and sensitivity of three kinds of method for differentiating the source of the hematuria were analysed and compared.Results Among the three kinds of methods,diameter measurement was significantly superior to other two kinds of method in specificity and in sensitivity.Conclusions It is suggested that diameter measurement of urinary red blood cells be reliable for differentiating the source of the child hematuria.This method is easy to operate and fit to spread in basic hospital.
