重建钢板内固定术治疗肩胛骨骨折的研究

目的：探讨重建钢板内固定术治疗肩胛骨骨折的方法及临床效果。方法：将68例肩胛骨骨折患者随机分为治疗组和对照组各34例,其中对照组用传统的三角巾悬吊及牵引等保守疗法进行治疗,治疗组组采用重建钢板内固定术进行手术治疗。结果：两组患者均全部获得随访,时间6～36个月。随访结果显示,治疗组优良率为88.2%,同时仅发生愈合畸形1例,并发症发生率2.9%;对照组优良率79.4%,同时发生有创伤性关节炎及愈合畸形各2例,并发症发生率11.8%,两组患者优良率及并发症发生率情况比较均有统计学意义（P〈0.05）。结论：对于肩胛骨骨折,通过CT三维重建进行早期诊断,并合理确定手术入路、把握手术适应证、科学选择内固定材料后,与传统保守治疗比较更能达到良好的复位、固定以及尽快促使骨折恢复,同时也更能减少创伤性关节炎及愈合畸形等并发症的发生,其临床效果满意,值得临床推广应用。

Study on the Reconstruction Plate Internal Fixation in the Treatment of Scapula Fracture

Objective：To study the method and clinical effect of reconstruction plate internal fixation in the treatment of scapula fracture.Method： 68 patients with scapula fracture were randomly divided into treatment group and control group with 34 cases in each group,the control group were treated with conservative treatment such as conventional suspension with triangle bandage or traction therapy,the treatment group were treated with reconstruction plate internal fixation.Result：The two groups were all followed up,the time was six to thirty-six months,the excellent and good rate of treatment group was 88.2%,and 1 cases was with healing deformity,the incidence of complication was 2.9%,the excellent and good rate of control group was 79.4%, 2 cases were with traumatic arthritis and healing deformity,the incidence of complications was 11.8%,the incidence of complications and excellent and good rate of two groups had significant differences（ P ＆lt;0.05）.Conclusion：The patients with scapula fracture should be early diagnosed with three-dimensional reconstruction of CT,confire the operative approach,master the operation indications and select the internal fixation material,and it has better restoration and fixed effect to the conservative treatment,so it can quickly promote the recovery of fracture,and it can more effectively decrease the incidence of complications such as traumatic arthritis and healing deformity,it’s clinical effect is satisfacted and is worthy of clinical popularization and application.
