股骨近端抗旋髓内钉与锁定钢板治疗股骨粗隆间骨折的疗效比较

摘    要:目的:比较股骨近端螺旋刀片抗旋转髓内钉( proximal femur anti-rotation intramedullary nail,PFNA)与股骨近端锁定钢板( proximal femur locking plate,PFLP)治疗股骨粗隆间骨折的疗效。方法回顾性随访2012年1月—2014年1月接受手术治疗的40例股骨粗隆间骨折患者,手术方式随机选择,其中PFNA治疗15例,PFLP治疗25例。比较两组手术时间、术中出血量、下地行走锻炼时间、术后并发症、骨折愈合时间。结果两组患者在手术时间、术中出血量、下地行走锻炼时间方面,P<0.05,有统计学差异,PFNA治疗组优于PFLP治疗组;在骨折愈合时间、术后并发症方面,P>0.05,无统计学差异。结论 PF-NA治疗股骨粗隆间骨折具有手术时间短、术中出血量少、下地行走锻炼时间早等优点。

Abstract:Objective To compare the effects of proximal femur nail antirotation ( PFNA) and proximal femur locking plate ( PFLP) in the treatment of femoral intertrochanteric fractures.Methods Retrospectively analyzed 40 cases of intertrochanteric fracture patients in our department from January 2012 to January 2014,with 15 cases treated with PFNA and 25 cases with PFLP.The operation time,a-mount of bleeding were compared between the two groups,and the walking exercise time,postoperative complications,fracture healing time were also compared.Results The operation time,bleeding volume,and walking exercise time of these two groups had significant difference (P<0.05).The PFNA group had better results than the PFLP group,but there was no statistical difference in fracture heal-ing time and postoperative complications in the two groups ( P>0.05).Conclusion PFNA showed shorter operation time,less bleed-ing,and more early walking exercise time etc.
