外伤性支气管断裂的诊治

目的：探讨外伤性支气管断裂损伤的诊断及外科治疗。方法：采用回顾性分析法对1994年1月-2007年12月华西医院收治的16例外伤性支气管断裂损伤患者的临床资料进行分析。结果：本组中一期行支气管端-端吻合术9例，行支气管修补术3例；3例行左全肺切除术，1例先行保守治疗，后因裂伤部位狭窄明显而行狭窄部袖式切除支气管端-端吻合术。本组均痊愈，无手术死亡病例。术后15例获随访1～13年，3例有轻度狭窄，但不影响生活和工作。15例生活质量均好。结论：外伤性支气管断裂损伤易误诊或漏诊，胸部CT及纤维支气管镜检查有助于早期诊断。外伤性支气管断裂损伤一经确诊，尽早施行支气管修复重建术。

On Diagnosis and Surgical Treatment of Traumatic Bronchial Rupture

Objective： To investigate the diagnosis and surgical treatment of traumatic bronchial rupture. Methods： Making a retrospective analysis on the treated 16 cases of patients of traumatic bronchial fracture injury from Jan. 1994 to Dec, 2007. Results： 9 patients performed bronchial end-to-end anastomosis, 3 bronchial repair, 3 left pneumonetomy, 1 conservative treatment firstly and resection was performed due to bronchial narrowing sleeve. No operative deaths. 15 cases were followed up 1-13 years and three cases have mild stenosis which do not affect life and work. Conclusion： All patients have good quality of chest CT and fiber bronchoscopy contribute to early diagnosis. If thetrauma of the bronchial is diagnosed, it is best to repair and reconstruct the bronchics as soon as possible.
