食管癌附膜内支架置入术联合放疗疗效分析

目的　通过回顾比较食管附膜内支架置入术结合放疗和单纯支架置入术治疗食管癌疗效。评价附膜内支架置入结合放疗对食管附膜内支架置入术后再狭窄的预防和治疗价值。方法　 2 0 0 1年 1月— 2 0 0 4年 1月 ,对 64例食管癌患者中 ,3 0例行食管附膜内支架置入治疗 (对照组 ) ,3 4例行附膜内支架置入结合放疗 (观察组 )。治疗前均行食管镜检查 ,并取得组织学诊断。全部跟踪随访观察疗效。结果　 3 0例单纯食管附膜内支架置入病人有 2 4例食管再狭窄 ,2 7例 ( 16例淋巴结转移 ,胸壁转移 4例 ,肺转移 5例 ,骨转移 2例 ) ,3年生存率 13 .3 % ;3 4例附膜内支架置入结合放疗的病人有 10例食管再狭窄 ,18例转移 ( 16例淋巴结转移 ,胸壁转移 1例 ,骨转移 1例 ) ,3年生存率 47.1%。结论　晚期食管癌附膜内支架置入结合放疗对食管再狭窄的预防和治疗有一定的疗效。

Effect of Patients with Esophageal Carcinoma Treated by Covered-metallic Stent with Radiotherapy

Objective To observe the therapeutic effect of Covered-metallic stent combined with radiotherapy and by Covered-metallic stent alone in the treatment of esophageal carcinoma and to assess the value of Covered-metallic stent.Methods From 1998.11 to 2001.1,In 64 patients,30 patients were treated by Covered-metallic and 34 patients were treated by Covered-metallic with radiotherapy.Results 30 patients were treated by Covered-metallic alone,esophageal strictures cases were 24 and distant metastatic cases was 27. 3-year-survival rate was 13.3%;34 patients were treated by Covered-metallic with radiotherapy,esophageal strictures cases were 10 and distant metastatic cases were 18. 3-year-survival rate was 47.1%.Conclucion Covered-metallic with radiotherapy is definitely effective in the prevention and treatment of esophageal strictures for the patients with Covered-metallic alone.
